Diamond Grove MYPAC

Mississippi Youth Programs Around the Clock (MYPAC) is a home and community-based Medicaid outpatient program for children and youth ages 6-21 with Serious Emotional Disturbance (SED), that utilizes the Wraparound process.

What is Wraparound?

Wraparound is a planning process that helps ensure that the child/youth will remain in their homes and communities. With help from one or more facilitators, the family and its support system will work together to coordinate their activities and blend their ideas regarding the family’s needs.

Services include:

  • Referral to additional community resources who may be needed to assist the child and/or family.
  • The child and family will work with a facilitator to create a team for informal and formal support. The team can include: The family, family members, neighbors, friends, coaches, church members and the treatment team.
  • The family’s team will develop one plan based on the strength of the child and family.
  • Transition planning with the aim of ensuring the family can manage when the formal process of wraparound ends.

What can I expect?

A facilitator works with the family to identify people who know the family best and then engages those people in the wraparound process. These people form a support team that works together to achieve the goals chosen by the family.

The child/family always has:

  • A choice about the services they receive
  • A voice in the manner in which they receive services
  • Ownership of decisions that affect their lives

Who makes referrals?

Typical referral sources include but are not limited to:

  • Mississippi Child Protective Services
  • Division of Medicaid
  • Community Mental Health Centers
  • Acute Care Hospitals
  • Residential Treatment Centers
  • Youth Court Counselors
  • Guardian ad Litems
  • Private Practitioners
  • School Districts (counselors, principals, teachers)
